Diabetes Management Centre

Our goal is to support those living with Prediabetes, Type 1, Type 2 or Gestational Diabetes to feel confident in self-managing this disease. We will teach clients about necessary skills for a healthy lifestyle including controlling blood sugars, eating a balanced diet and keeping active. 

About Us

Who do we see?

The Diabetes Management Centre sees adults (ages 18+)  living with Type 1, Type 2 or Gestational Diabetes as well as those who  have Prediabetes.


Referrals from physicians and self-referrals are accepted.

Who are we?

We are a team of Certified Diabetes Educators including Registered Nurses and Registered Dietitians who work alongside Diabetes specialists (Endocrinologists) to help you manage  your Diabetes.

What do we do?

Services we offer:

  • Individual appointments with a Nurse and/or Dietitian
  • Group sessions – cooking demos, education and exercise classes
  • Specialist care as needed if referred to by a physician
  • Education and support for adults controlling their diabetes with medications, injections, insulin pumps or simply with diet and lifestyle
  • Insulin pump starts and support
  • Ongoing follow up as needed by clients

Diabetes is a chronic disease that happens when your blood sugar, also called blood glucose, is too high. 


Blood glucose is your body's main source of energy and comes from the food you eat. Insulin, a hormone made by the pancreas, helps move the glucose from food into your cells to be used for energy. Sometimes your body doesn’t make enough—or any—insulin or doesn’t use insulin well. Glucose then stays in your blood stream and can affect other organs. Although diabetes has no cure, you can take steps to manage your diabetes and stay healthy.


There are four types of diabetes:

  • Type 1 Diabetes (the body cannot make any insulin)
  • Type 2 Diabetes (the body does not make enough insulin or doesn't use it well enough)
  • Gestational Diabetes (the body does not make enough insulin during pregnancy)
  • Prediabetes (blood sugars are higher than normal but not high enough to be considered Type 2 Diabetes)


More than 9 million Canadians live with Diabetes or Prediabetes. Many  don't even know they have it - chances are diabetes affects you or someone you know. No matter what type of diabetes you have, the  educators in the Diabetes Management Centre will help you manage your diabetes and prevent or delay complications. 


Referrals from physicians and self-referrals are accepted.


Physician Referral:
Physicians can refer their patients to this service by faxing the 'Physician Referral Form' form found below (fax: 705-719-4877).  


Self-Referral:
Self-referrals are accepted and can be dropped off in person or submitted online. 

To self-refer online, please find the 'Online Self-Referral Form' link below.


Once a referral is received, our reception will contact you to arrange an appointment.


 Some of the topics your Registered Nurse and/or Registered Dietitian will talk with you about are:

  • How to monitor your blood sugars
  • Ways to manage your blood sugars
  • Healthy eating and weight management
  • How your Diabetes medications work
  • Eating in special situations (i.e. dining out, parties)
  • How to manage Diabetes when you're sick or when you travel
  • Diabetes and pregnancy
